Information about enrolling in COBRA, which allows you to continue your coverage beyond this date, will automatically be sent to your home address from the New York State Department of Civil Service, Employee Benefits Division. If you would like to enroll in COBRA, you will have 60 days after the date of notification to return the forms to the Department of Civil Service.
It is important to update your address so that you may receive health benefits correspondence.
If all summer monies are not collected in your final spring paychecks:
If you have a summer appointment, it is possible your summer payments may have deductions. If this happens, you will receive a credit.
If your employment terminates, your eligiblity for coverage ends. Use of a benefit card when you are not eligible may constitute fraud.
