Faculty Request to Extend SEAS Start Up Commitment

This form should be submitted by SEAS faculty who are seeking to extend the expiration date of their SEAS start up commitment. This form is only in reference to the SEAS commitment. Commitments made by other sources need to be discussed with those funding sources (for example: department provided start up must be discussed with your department). This form must be submitted no later than six (6) months prior to the expiration date on the commitment.
