All students enrolled in coursework at UB after January 1, 2011 (spring 2011 semester) must request official transcripts VIA their HUB Student Center. However, if additional documentation is required to be submitted concurrently with your transcript (LSDAS report, statement of Rank) or if you are sending an expedited UPS label, please continue to use the Law School Transcript Request Form (PDF) and drop off, mail, fax (716-645-6378) or email (email@example.com) the forms together to Records and Registration in 304 O'Brian.
Students whose last enrollment was prior to Jan. 1, 2011
(fall 2010 semester) should use the Law School Transcript Request Form (PDF), as
you do not have access to the HUB Student Center. If additional

together to Records and Registration in 304 O'Brian.
Records for the schools of Undergraduate/Graduate/Pharmacy, Medicine & Biomedical Sciences, and Dental
Medicine are maintained in those schools. If you are a
current or former undergraduate, graduate, or pharmacy student,

Beginning June 20, 2011, all students enrolled in coursework at UB after January 1, 2011 will be able to print their own unofficial student transcripts via their HUB Student Center. Records and Registration will no longer accept requests for unofficial transcripts from students who have such access. Requests for unofficial copies of transcripts for students who do not have access to the HUB Student Center will be honored by filling out the Law School Transcript Request Form (PDF).
We are not permitted to provide transcript services to any current or former student who owes money to the University (tuition/fees, penalties, parking fines, library fines, etc), as designated by the service indicator codes below. Additionally, we cannot release transcripts for students whose names appear on the NYS Student Loan Service Center Loan Default list.
Official transcripts will not be released for students who have university financial obligations, indicated by BAG (No Access to My Account Info), BCA (No Access to My Account Info), BIL (Unpaid Bill), BJD (No Access to My Account Info), BNT (No Access to My Account Info), BPW (No Access to My Account Info), BWO (No Access to My Account Info), DEN (Dental Medicine), FIN (Department Financial Obligation), ILN (Institutional Loan Issue), LAW (Law School), and PAR (Parking Ticket) Service Indicators.
Proof of a pre-paid shipment order is required for expedited delivery service, and must be received at the same time as the transcript request form. If it is not received, the transcript will be sent via regular mail.
Please visit http://www.ups.com and click on "Create a Shipment" to obtain proof of a pre-paid shipment. In each instance you will need to register to access the site. Please select the location as "US," package type as "letter" and weight as 0.1 lbs. Make sure you fill in your own address and credit card information for billing, and then OUR address for the return address so the recipient of your transcript knows the package is coming directly from our office. The "send to" address should be the location you wish your transcript delivered to. You will receive a PDF version of your pre-paid shipping label, which should be sent with your transcript request form to our office via fax at 716-645-6378 or email at email@example.com.
Our staff works very hard at processing your requests in a timely manner, however, please give us as much notice as possible if you have specific deadlines to meet. Typical turnaround time for a transcript is 1-3 business days, but this is lengthened during peak times such as exams, registration, and degree conferrals.
Students may need to apostille or legalize their transcript and/or diploma for use in a foreign country. The Office of the Registrar assists students with this process by notarizing the document, and coordinating with the offices of the Erie County Clerk and New York State Secretary of State to complete the apostille.
